30 뼈의 발생학 골형성 : 막뼈 발생, 연골뼈 발생 임신 10 주경 전체적인 형태 갖춤 초음파 관찰 : 8 주 - limb bud 9 주 - 몸의 움직임 관찰 11 주 - 손, 발가락 14 주 - 대부분의 심한 골격계 이상 관찰 17 주 -distal sacrum

31 뼈의 형성 뼈는 mesenchymal condensation 에서 시작. 막뼈 발생 : condensation-connective ts. ;skull, facial bone, clavicle, mandible 연골뼈 발생 : condensation-cartilage. ;most bones

33 골격계 초음파 장골 : 길이 (2SD), 모양, 골화 정도 머리뼈 : 모양, 골화 정도 얼굴 : frontal bossing(Achondrogensis, TD, achondroplasia) 흉곽 : 흉곽의 크기 (5th %) 및 모양, 늑골골절 척추 손과 발 : 발바닥 길이 (Fl/foot length < 0.9)

34 흉곽의 모양 - 흉곽은 복부쪽으로 점차적으로 종모양 (bell shape) 으 로 커진다. -Thanatophoric dwarfism.: 특징적으로 좁은 흉곽 -Osteogenesis imperfecta.: 다발성 늑골 골절 - 흉곽둘레 (TC)/ 복부둘레 (AC): 0.89+/-0.06 > 0.8 태령 20 주 이후 불변

41 Thanatophoric dysplasia. I 0.27~0.4/10,000( 치명적인 골격계 이형성중 가장 흔함 ) FGFR3 이상 Type 1(85%): femur-telephone receiver ACC(25%) Type 2(15%): clover leaf skull

42 Thanatophoric dysplasia. II 초음파 소견 : 심하게 짧은 사지 clover leaf skull/ bowed long bone flattened vertebral body 이마가 튀어나옴 좁은 흉곽 및 양수과다 골화 정도는 정상, 또는 약간 감소

43 Achondrogenesis. I 1/40,000~50,000( 두 번째 ) Type 1(20%): 머리뼈, 척추, 골반의 골화가 안됨 다발성 늑골 골절 Type 2(80%): 골화 정도 정상 늑골 골절이 없다.

44 Achondrogenesis. II 초음파 소견 : 심하게 짧은 사지 경부 투명대의 두께 증가 태아의 움직임이 적다. 골화 장애 및 늑골 골절 (1 형 ) 태아 수종, 양수 과다증

45 Homogenous Achondroplasia 대개 부모가 모두 연골 발육 부전일 때 발생 하며 적어도 한 쪽은 이환 되어있다. 흉곽이 좁으며, thanatophoric dysplasia 와 유사한 소견

46 Osteogenesis imperfecta 산전 진단은 대개 type 2 초음파 소견 : 골절과 callus 형성 사지가 단축되고 변형된다. 머리뼈 골화 장애로 뇌구조가 잘보임 흉곽이 좁고 늑골 골절 예후 : 치명적

47 Otopalatodigital syndrome type II The findings included: 1. Large, bowed bones mostly at the level of the tibias with very small fibulas. 2. Hypertelorism, micrognathia 3. Narrow thorax 4. Feet and hands deformities: toes separation with hypoplastic large toe on one side and missing on the other. 5. Platyspondyly.

51 Campomelic dysplasia Campomelic dysplasia Synonyms : campomelic syndrome, campomelic dwarfism, congenital bowing of the limbs. Definition :Campomelia is a syndrome featuring angulation of the long bones such as femur and tibiae. Some authors have classified the disease into two varieties: “ long limbed ” and “ short limbed, ” depending on the type of limbs involved in the pathological process

52 Pathogenesis 1) mechanical stress due to faulty fetal position within the uterus 2) primary muscle imbalance and shortening, particularly of the calf muscles causing secondary bending of the tibia 3) intrauterine fracture with subsequent healing 4) abnormal vascular and cellular elements of perichondrium 5) developmental disturbance in the cartilagineous phase of bone formation.

53 Associated anomalies : 1. High forehead with prominent occiput 2.low-set ears, micrognathia, hyper ­ te ­ lo ­ rism 3.cleft of the soft palate, small and bell-shaped chest, 4.short limbs, hypoplastic fibulae, eleven pairs of ribs, 5. hypoplasia of the scapulae, scoliosis or kyphoscoliosis with hypoplasia of the midthoracic vertebral bodies, 6.high and narrow pelvis, dislocation of the hips, 7.talipes equinovarus and cutaneous dimpling, 8.soft and reduced tracheobronchial cartilages. 9.Central nervous system, cardiac and renal abnormalities
