4 Erectile dysfunction 란 ? ▶ What is the ‘Erectile Dysfunction’? - 발기부전은 성생활에 충분한 발기가 되지 않거나 유지되지 않은 상태를 의미. 일반적으로 이러한 상태가 3 개월 이상 지 속되었을 경우 발기부전으로 정의. ▶ The cause of Erectile Dysfunction? PastPresent - 발기부전의 원인이 대부분 심인성 이며 기질적인 원인들은 단지 일부의 경우에만 해당 - 기질적인 원인들이 발기부전의 흔한 원 인으로 밝혀짐 - 특히 50 세 이상의 연령층에서 심혈관 계 질환과의 관련성이 두드러지게 보임

11 Words # cGMP(Cyclic guanasine 5’-phospate) - 발기물질. 발기하려면 cGMP 라는 핵산화합물이 필요. 이 물질은 성기의 동맥을 확장시켜 혈액이 성기로 몰리게 해줌. # PDE5(Phosphodiesterase) -PDE5 라는 효소가 발기물질인 cGMP 를 파괴하면 발기했던 성기가 수그러 든다. 발기부전 환자들은 이 효소가 왕성하게 작용하기 때문. # 11 종류의 효소 -cGMP 를 파괴시키는 PDE 라는 효소는 우리몸 안에 무려 11 종류가 존재 (PDE-1~PDE-11) 이 효소들은 우리 몸 곳곳에서 필요한 일을 수행.

13 작용원리 치료제의 작용원리 -Sildenafil 은 우리 몸에 흡수되어 남성이 성적으로 흥분할 때 생성되는 ‘cGMP’ 라는 화학물질의 분비를 돕는 동시에 발기 저해 물질인 PDE5 (Phosphodiesterase) 를 분해 -cGMP 는 혈관확장물질인 NO(nitric oxide) 를 생성하는데 이러한 일련의 과정이 남성의 성기부분에 보다 선택적으 로 작용하게 되어 발기부전에 탁월한 효과

17 Abstract The SAR of a series of β-Carboline derived type 5 phosphodiesterase inhibitors has been explored and we have discovered compounds with excellent levels of PDE5 potency and selectivity over PDE6. However, the series exhibits low levels of selectivity over PDE11, a phosphodiesterase with unknown function.

18 Introduction Secondary messenger cyclic nucleotides -Adenosine cyclic 3’,5’-phosphate(cAMP) -Cyclic guanasine 5’-phospate(cGMP) cGMP specific PDE5 enzyme controls the levels of intracellular cGMP and plays a critical role in its modulation, duration of action and physiological function. - PDE5 is the major PDE isozyme in penile corpus cavernosum tissue and plays a key role in the control of penile erection.

19 Introduction -During sexual stimulation nitric oxide(NO) is released -Activates guanylate cyclase to form cGMP via cyclisation of GTP. -Resulting in stimulation of cGMP synthesis, which, in turn, reduces intracellular calcium following binding to cGMP- dependant protein kinases and cGMP-dependant ion channals.

20 Introduction -Sildenafil & Vardenafil, show excellent PDE5 selectivity over PDE11. Whereas Tadalafil exhibits low selectivity. -PDE11 inhibition is not clearly understood but could have a role in sperm function. -In the future, PDE11 inhibitors might be explored for novel indications relating to hormonal or other functions, but when developing PDE5 inhibitors to modulate vascular function it was thought desirable to maintain high selectivity of PDE5 effects over PDE11.
