4 기준 이식대기자 그 중 신이식의 대기자 수도 해마다 증가하여 2010년에는 10561명에 이르고 있다. 신이식의 수요의 증가에 따라 예전에는 공여자로 부적합하다고 판단했던 뇌사자에 대해서도 이식을 고려하고 있다. 선택 기준을 넓힌 확대된 공여자 선택기준 즉 expanded donor criteria 를 선택하여 공여자 부족을 해결하려는 시도이다. 2010 KONOS annual report

8 이상적인 신이식 공여자 선택의 기준 나이 16-45세 냉허혈 시간 24시간 이하 전신적인 감염 (-) 악성 종양 (-)
당뇨, 고혈압 (-)

9 Expanded donor criteria - UNOS
공여자의 나이가 60세 이상인 경우 나이가 50세에서 59세이면서 고혈압이 있거나, 뇌사의 원인이 뇌혈관 질환인 경우 장기 적출 전 마지막 혈청 크레아티닌이 1.5 mg/dL 이상인 경우  이중 두가지 이상을 만족하는 경우

공여자의 나이가 60세 이상 심정지 공여자 저혈압이 3회 이상 혈청 크레아티닌 3mg/dL 이상 또는 크레아티닌 청소율이 60mL/min 이하 2회 이상의 소변검사에서 단백뇨가 (++) 이상  이중 한가지 이상을 만족하는 경우

Standard-Criteria Donor 35-yr-old man who has no history of hypertension or diabetes and for whom the cause of death is a motor vehicle accident who do not meet any of the criteria for an ECD cJASN 4; ,2009

Donation after Brain Death whom cardiac circulation and respiration remain intact maintained by medical measures, including mechanical ventilation,drugs, intra-aortic balloon pump, or extracorporeal machine oxygenation device cJASN 4; ,2009

donor who does not meet the criteria for brain death but in whom cardiac standstill or cessation of cardiac function occurred before the organs were procured non–heart-beating donor Controlled DCD a donor whose life support will be withdrawn and whose family has given written consent for organ donation in the controlled environment of the operating room Uncontrolled DCD a candidate who expires in the emergency room or elsewhere in the hospital before consent for organ donation is obtained and catheters are placed in the femoral vessels and peritoneum to cool organs until consent can be obtained cJASN 4; ,2009
