3 Delphi- Dysfunctional Tear Syndrome

4 DTS with lid margin disease, Tear distribution을 따로 분류하여 원인별 치료 지침이 따로 정해진 장점이 있는 반면 Dry eye component에 대한 치료지침은 DTS without lid margin disease에만 한정되어 있음

5 Level of severity를 정하는 Symptoms & Signs의 모호한 구분
mild symptoms? moderate corneal staining? 고식적인 NEI classification, 즉 aqueous deficient or evaporative 의 분류법이 포함되어 있지 않음.  Severity LEVEL에 Schirmer, TFBUT의 영향 없음

Discomfort & visual symptoms level 객관적 구분 모호 원인에 따른 분류에 상관없이 severity level에 따른 치료방침  MGD, ocular surface disease 등의 상이한 병인데 대해서 모두 동일하게 적용됨

건성안 환자 Study 대상자 Patient Inclusion & Exclusion 1) Inclusion criteria 건성안 관련 증상 호소 - 이물감, 따가움, 흐림, 통증 등 최소 20세 이상의 성인 최대 교정 시력 0.2 이상 2) Exclusion criteria 약물 치료가 필요하거나 치료중인 감염성 질환, 알러지성 결막염, 각막미란, 각막상피병증 등 우선적인 치료가 필요한 안질환 - 외상, 각막미란, 공막염, 포도막염, 안내염, 급성녹내장, 황반변성 등 협조가 불가능한 환자

22 6 Conjunctival staining
One drop-wetted Lissamine green strip을 inferior palpebral conjunctiva에 묻힌 후 수차례 눈감빡임 리사민그린 점적 후 최소 1분 후에 결막의 등급을 측정 Oxford scheme 기준, Conjunctiva를 temporal/nasal 2영역으로 나누어 각 영역을 grade 0-5 척도로 개별적으로 등급 시약 부재 및 문헌상 결막염색 numeric scale의 구분이 모호하므로 LEVEL구분은 DEWS 기준에 따르고 Oxford기준의 grade는 표기만 할 것 LEVEL 1 LEVEL 2 Conjunctival staining (DEWS, 2007) none to mild LEVEL 3 LEVEL 4 + moderate ++ marked to severe Lissamine green conj. staining
