2 Attachment는 초기에는 지식과 경험 부족으로 신비하고 특수하고 환상적인 장치로 여겨짐

3 Definition of attachments
치과 보철물의 fixation, retention, stabilization을 위한 기계적 장치이다. RPD에 있어서 이는 clasp대신 direct retainer로써 작용한다

4 Direct retainer로써 작용시 Support – resistance to movement of the prosthesis toward the tissue Retention – resistance to movement of the prosthesis away from the tissue Reciprocation – counteraction of the forces exerted by the retentive component

5 Direct retainer로써 작용시 Stabilization – resistance to horizontal movement of the prosthesis Fixation – resistance to movement of the abutment tooth away from the prosthesis and movement of the prosthesis and movement of the prosthesis away from the tooth Terminal portion에서는 passive한 형태

By fabrication method, tolerance of fit Precision – prefabricated, machined components with precisely Semiprecision – less precise tolerance, manufactured patterns, hand made By relationship to abutment tooth Intra-coronal attachment

By relation to abutment tooth Intra-coronal attachment : internal attachment 치아 장축에 힘이 전달되며 수직, 측방력에 더 잘 견딘다. Extra-coronal attachment : external attachment 지대치 삭제를 줄인다

8 Classification of attachments
Rigid type – no movement of components during function (아무리 좋은 조건에서도 미세한 운동이 발생, 구성 부품에 마모 발생) Resilient type – 기능중 의치 상하방 조직으로의 한정된 운동과 이동을 허용하므로 이론적으로 최소한의 힘을 지대치에 가한다

9 Intracoronal rigid attachment Extracoronal resilient attachment
Direct retainer의 기능을 충분히 발휘 Extracoronal resilient attachment 적절한 support와 bracing을 제공하지 못한다. Surveyed crown 상에 rest와 guiding plane설정을 필요로 한다. 이는 framework의 fit 정도와 residual ridge에 대한 denture base와의 positive relationship을 제공해 준다

11 Deciding on attachment use
Esthetic Divergent abutment teeth with high survey line – conventioanl clasp arm 사용시 HOC를 낮추기위해 tooth modification이나 surveyed crown이 필요하다. Attachment 장착을 위해서는 crown제작이 필요하지만 지대치간의 parallelism에 구애받지 않는다. RPD의 path of placement는 crown자체의 parallelism보다는 attchment장착 path에 의 결정됨

Major advantage Lever arm을 짧게하므로 occlusal, incisal rest 보다는 지대치에 가해지는 force를 apically하게 유도하며, torquing force를 줄인다 Better cross arch force transmission and stabilization than clasp (type, guiding plane 수 framework와 attachment의 design and adaptation에 의해 결정 된다.

Negative aspects Additional expenses Poor dental motivation과 manual dexterity 결여로 조기 실패 Repair, alternation이 어렵거나 불가능 하다

Contra-indication Short clinical crown Minimum 4mm of vertical space for attachment Minimum 6mm of clinical crown (이하인경우 attachment부착을 위해 overcontour가 필요) Occlusogingival length는 occlusal plane 과 적절한 space를 요구하며gingival portion에도 space를 필요로 한다. Friction resistance를 얻기위해서도 필요

15 Deciding on attachment use
Contra-indication Short faciolingual width/improper anatomic contour 가철성의치의 보조적 유지 강화장치 즉 lock이나 retentive clasp arm장착을 위해서 필요하며, 절치 견치에서는 협설폭경이 협소하여 attachment 장착의 제한을 받게 된다.

Contra-indications Inadequate space between the pulp and the normal contour of tooth Internal attachment장착시 제한을 받는다. RC tx Poor periodontal health of abutment, poor tissue quality, poor C/R, endo & restorative consideration

Distribution of occlusal stress Residual ridge상의 최대한 cover : denture base Support, bracing, retentio, direct-indirect retention, stability의 확보

18 Application of prosthodontic principles
Framework와 denture base, tooth간의 적절한 관계 설정 Rigid framework 필요 최소 3개 치아 이상과 접촉 2개 지대치 접촉시 additional contact이 필요하다 Attachment기능을 deactivate시키고 rigid prosthesis상에서 base와residual ridge관계를 평가할 필요가 있다.

Non-rigid, stress-directing attachment Rigid type사용: mucostatic impression technic필요, 지대치에 torque force damage우려 Stable base precision attachment concept : (floating denture base concept)- rigid internal attachment, cast metal base with mucostatic impression, male portion은 denture base에 연결 tissue ward loading시에만 지대치내로 완전 seating

21 Attachment selection Kennedy Class V
Edentulous ridge에 심한 defectrk 있을 경우 Tissue bar type이 좋다 Bilateral distal extension인경우 tissue bar는 straight, flat, fulcrum line에 parallel, sagital plane에 수직으로 설치 (rotation movement를 허용) – stud type은 안된다 Guiding plane은 운동과 관계없이 설정
