2 Chief complaint 위궤양의 치료 위해 onset : 내원 1년여 전

3 Present illness 26세 여자 환자로 2004년 3월 심한 상복부 동통과 속쓰림을 주소로 외부병원에서 검사 시행
2004년 3월 심한 상복부 동통과 속쓰림을 주소로 외부병원에서 검사 시행 EGD – GU (PW of antrum) Helicobacter pylori (-) - CLO, serology, biposy Biopsy – negative Gastrin level pg/ml (<100pg/ml) 1년 정도 여러 종류의 PPI를 이용하여 치료하였으나 궤양이 치유되지 않아 전원 됨. 동반 증상> 체중감소 (7~8kg/1yr), 복통 및 속쓰림

General : not so ill-looking appearance HEENT : anemic conjunctiva, clear sclera Chest : clear breath sound without crackle regular heart rhythm without murmur Abdomen : soft and flat, normoactive bowel sound tenderness/rebound tenderness(-/-) hepatomegaly(-) splenomegaly(-) Back & Extremities no pretibial pitting edema, no CVA tenderness

- Genotype : CYP2C19 *1/*1 Phenotype : Ultra-extensive metabolizer  PPI제제를 rabeprazole 로 변경하여 double dose (40mg/d)를 8주간 사용함.
