2 Case 유 O 영 F/25 C.C.: PBSCT (Peripheral Blood Stem
Cell Transplantation) for AML P.Hx. : no known Pul tbc, hepatitis DM, hypertension FHx. : N-C 유지영 25세 여환입니다 상기 여환은 peripheral blood stem cell transplantation위해 내원하였습니다 과거력 및 가족력상 특이 소견 없었습니다

3 P.I. : 상기 여환 은 2004년 4월 혈액검사상 pancytopenia 소견으로 시행한
골수검사상 Acute Myelocytic Leukemia (M2) 진단되어 cytosine arabinoside, idarubicine 으로  1st induction chemotherapy 받고 1st remission 판정받았으나 한달 후 재발 소견 보여 Glivec (400mg/day) 3 개월 복용 후 2nd remission 소견 보여 동종 말초 조혈 모세포 이식 (PBSCT) 시행 예정으로 Baseline study위해 내시경 시행함.

ROS : General weakness / Easy fatigability (+/+) Cough / Sputum (-/-), Dyspnea / chest pain (-/-), Abdominal pain (-) Dysphagia/ Anorexia / Nausea / Vomiting (-/-/-/-) Melena / Hematochezia (-/-) Wt loss / Wt gain (-/-) P/E : BP 110/70 mmHg, PR 76/min RR 21/min, BT 36.7℃ Chonic ill looking apperance Slightly pale conjunctivae Clear breathing sound Regular heart beat without murmur Soft and flat abdomen without tenderness 환자는 general weakness 및 esay fatigability를 호소하였으며 P/Ex상 chronic ill looking appearance 및 conjunctiva는 pale하였습니다

5 Megakaryocyte are adequate, Blasts < 5% of TNCs,
Lab. : CBC 5,470/13.6/236,000 BUN/Cr 17.8/0.9 mg/dL, AST/ALT 7/14 IU/L T.pro/Alb 7.2/4.9 g/dL, T.bili 0.4 mg/dL LDH 240 IU/L Other studies BM Study ( ): AML-CR Hypocelluar marrow (20~30%), Megakaryocyte are adequate, Blasts < 5% of TNCs, immature cells are rarely seen 유전자 검사: minor bcr/abl (+)  Lab상 특이 소견 없었고, 1st 재발 당시 BM study상 특이소견 없었으나 유전자 검사에서 minor bcr/abl 양성 소견 보였습니다.

6 EGD (D-11: ) PBSCT시행하기 위해 시행한 내시경 소견입니다.EGJ, body,antrum, duodenum에는 특이 소견없었고

7 Single polypoid lesion on posterior wall of upper body
Upper body posst wall 에 single polypoid lesion이 관찰되며 surface에는 whitish material 로 덮힌 소견이 관찰되었습니다

8 Endoscopic Diagnosis Gastric polyp in UB.PW R/O Leukemic infiltration
내시경 진단은 gastric erosion, R/O gastric polyp R/O leukemic infiltration으로 주었습니다

9 Pathology H&E stain x 40 x400 Dense infiltration of myleoblasts.
Pathology상 H & E stain에서 40 배율로 보면 inflammatory cell 침범 소견을 보였으나 400배로 확대한 결과 dense 한 myleoblast infiltation소견이 관찰되었습니다. H&E stain x x400 Dense infiltration of myleoblasts.

10 Busulphan/Cyclophosphamide Successful Engraftment
Flow sheet BMT Conditioning CTx: Busulphan/Cyclophosphamide D-0 F/U EGD BM Bx: Successful Engraftment D-6 D-11 D-2 D+14 D+21 EGD 환자 PBSCT 경과를 보면 D-11일 째에 EGD를 시행하고 D-6에서 D-2일 사이에 condition CTx를 시행하였고 D+14일 째에 BM Bx를 시행하여 successful engraftment를 확인하였고 f/u내시경을 D+12일에 시행하였습니다

EGD (D+21: ) Scarring change in previous lesion f/u 내시경상 UB PW부위에는 scarring change소견만이 관찰되었습니다

12 Final Diagnosis 1. Gastric granulocytic sarcoma (Chloroma)

13 Granulocytic Sarcoma A localized tumor mass composed of immature cells of granulocytic series 2–8 % with acute nonlymphoblastic leukemia(ANLL) Occurs concomitantly with or after or before the onset of ANLL Misdiagnosed as malignant lymphoma Common site: soft tissue, periosteum, bone ,LN, skin Associated with M1,M2 AML translocation t(8:21)(q22:q22) inversion of chromosome16 CD 56, Granulocytic sarcoma는 immature granulocytic cell이 local invarion하여 mass를 형성하는 것으로 acute nonlymphoblastic leukemia(ANLL)에서 2-8%발견되며 ANLL 진단시 혹은 후에 발견되며 아주 간혹 진단전에도 발견됩니다 .따라서 malignant llymphoma로 오인되기 쉬우며 흔히 침범하는 부위로는 soft tissue,periosteum, bone등르 침범하여 관련 인자로는 M1,M2 AML translocation t(8:21)(q22:q22), inversion of chromosome16, CD 56이 알려져 있었습니다

14 Granulocytic sarcoma Diagnosis: immunohistochemical techniques
positive for CD34, CD43,CD15, MPO, CD68 and occasionally CD45 negative for CD30 and CD20, ruling out the diagnosis of lymphoma Tx: standard daunorubicin and cytarabine induction therapy, followed by consolidation with high dose cytarabine 진단은 확진을 위해서는 immunohistochemical staining 을 시행하며 치료는 원인 질환에 대한 항암 치료를 합니다
