3 본 12개월된 남자환아는 내원 당일 처음으로 미음을 먹은 후 입과 목 주위에 생긴 두드러기를 주소로 내원하였다.

5 어머니가 임신 말기에 땅콩, 잣, 호두를 우유에 넣어 매일 한 잔씩 섭취하였다.

Family History Review of System Physical Examination Laboratory Finding Assessment Progress 1: breast milk 2: Korean rice 3: Japanese hypoallergenic rice 1(partially polished, Ichikawa Farm) 4: Japanese hypoallergenic rice 2(totally polished, Ichikawa Farm) 5: cooked (30 min) Korean rice 6: cooked (60 min) Korean rice 환아 엄마의 모유와 국내산 일반미, 일본산 저항원성 쌀 2가지, 즉 부분 도정된 쌀과 완전 도정된 쌀(Ichikawa Farm), 30분간 조리한 국내산 일반미, 60분간 조리한 국내산 일반미를 sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE)를 이용하여 단백분포를 알아보았고, 환아 혈청 내의 특이 IgE와 immunoblot을 시행함으로써 반응한 단백질을 시각화하였다. SDS-PAGE 상에서 모유, 국내산 일반미, 2가지 일본산 저항원성 쌀에서 kD에서 단백질이 관찰되나, 30분 또는 60분 동안 조리한 국내산 일반미에서는 관찰되지 않았다.(Fig. 2) 환아의 혈청과 반응시켰을 때 국내산 일반미와 일본산 부분 도정된 저항원성 쌀과 뚜렷이 반응하였고 일본산 완전 도정된 저항원성 쌀에도 약한 반응을 나타내었다. 그러나 모유, 30분 또는 60분간 조리한 국내산 일반미와는 반응을 나타내지 않았다.(Fig. 3)

15 Chief Complaint Present Illness Past History Family History Review of System Physical Examination Laboratory Finding Assessment Progress 환아는 우유 및 계란에 대하여는 제한식이를 지속하고, 이유식은 60분 정도 충분히 가열한 미음으로 진행하면서 피부 발진 및 두드러기의 발생은 더 이상 없었다.

16 5개월/여아 주소: 이유식 분유 한 스푼을 먹고 발생한 구토, 기면, 설사 검사 말초혈액 호산구수: 120/mm3 ,Total IgE: <18.0 IU/mL MAST: all (-) Skin prick test: 계란, 우유, 생선, 새우에 음성, 밀에 약양성 CAP: 우유, 콩, 계란 흰자, 땅콩, 밀, 쌀, 새우, 메밀 모두에 음성 Provocation (쌀미음): 수유 후 1-6시간에 걸쳐 구토, 청색증, 기면, 기관 지천명, 설사 등의 증상 유발시험 후 6시간에 시행한 상부 위장관 내시경 검사: 십이지장 구부 점막의 붉은 부종성 병변, 현미경 소견에서 아전 융모 위축 과 고배율 시야 당 약 10개의 호산구 침윤 소견
