2 Introduction -lactam 제제 - 작용점: penicillin binding protein (PBP)
- 4-member ring -lactam 구조 - 작용점: penicillin binding protein (PBP) 세포벽 합성 효소 - 감염증 치료에 가장 흔히 사용 - 대부분의 G(-) rod는 여러 종류의 -lactamase 생성 - 제3세대 cephalosporin 및 monobactam계 항균제 개발 - Extended-spectrum -lactamase (ESBL)

Imipenem, cefoxitin: not hydrolyzed CTX, CAZ, AZT: hydrolyze Inhibition of activity by -lactamase inhibitors 기존의 항생제 감수성 검사을 이용할 경우 PC, cephalosporin, aztreonam에 감수성인 균주중에 ESBL이 존재할 가능성 --- 임상적으로 내성

5 Current problem of ESBL
다른 균종으로의 내성 전달 가능성 - E. cloacae 등 Enterobacteriaceae 분리 빈도 증가: 세브란스 병원 CTX (R) K. pneumoniae % % % % ESBL 생성균은 모든 cephem 제제에 내성으로 간주되나, 일부 균주는 종전의 시험관내 감수성 기준으로는 이들 약제에 감수성을 보이므로 정확한 검출이 힘듦

9 CAZ/CA, CTX/CA Disk 준비 Clavulanic acid stock solution: 1000 ug/mL Plate에 CA 10 uL 를 ceftazidime (30 ug), cefotaxime (30 ug) disk에 점적 30분 정도 건조시킨 후 즉시 사용 냉동보관시 수개월간 안정

10 ESBL 선별기준 및 확인기준

11 Conclusion ESBL 검출을 위한 선별 검사 및 확인 검사가 필요함
ESBL 선별검사에서 양성인 경우 exclusion factor를 적용하여 확인검사를 시행하면 위양성을 줄일 수 있음 ESBL 확인 검사시 CAZ/CA와 CTX/CA를 동시에 사용하면 위음성을 줄일 수 있음
