3 What is the Cochrane Library?
1993년 영국에서 설립된 ‘The Cochrane Collection’에서 제공하는 Evidence-based medicine 분야의 Systematic Reviews 자료로 질병치료에 있어 객관적 임상실험 증거를 중심으로 한 치료효과 및 방법의 최신자료를 축적해 놓은 2차 정보원. 코크레인은  영국의 의사이자 역학자인 아키 코크레인(Archie Cochrane, 1909–1988)을 기념하기 위해 따온 명칭으로 그가 창안한 방법에 따라 창설된 기관이 ‘코크레인 공동연구 ( The Cochrane Library는  조사 가능한 모든 관련 Evidence를  수집하고,  체계적인 문헌고찰을 통해  분석해서 Healthcare treatments & Intervention의  효과성에 대한 전세계의 연구를  제공. The Cochrane Library는  임상의, 정책입안자, 연구자, 교육자, 학생 및 환자  모두 사용 가능한  데이터베이스. 

4 Who develops the Cochrane Library?
The Cochrane Library 의 수록 데이터 개발은 The Cochrane Collaboration에서 담당하고 있으며 Wiley를 통해 Publishing out. Wiley를 통해 전세계 서비스.  The Cochrane  Collaboration은 영국에 기반을 둔 국제적인 비영리 조직  Aim: 인류건강증진 How: Health Care에 대한 최상의 결정을 내릴 수 있도록 의료중재의 효과에 대한 Systematic Review들을 수집, 축적 및 홍보

5 The Cochrane Collaboration
Collaborative Review Groups : 주요 토픽들에 대해 Cochrane review를 준비하고 update하는 사람들과 이 과정에서 그들을 support하는 사람들로 구성. 53개의 리뷰 그룹은 Breast Cancer, Infectious Diseases, Tobacco Addiction등 각각의 토픽 별로 구성. Cochrane Centers : 전세계에 분포된 14개 센터는 이용자 교육, 번역사업 등 지역적, 언어적인 지원을 담당. Fields/Method Groups/Consumer Network 등의 하부구조로 구성. 한국은 2009년 9월 고려대학교 안암병원 내 Cochrane Center 오픈. 센터장: 고려대학교 의과대학 예방의학교실  안형식 교수. 한국 Cochrane Center는  호주 센터의 branch로 편입.

7 What can you do with The Cochrane Library?
Use: The Cochrane Library는 실제 입증된 유효한 증례에 기반을 두고 있기 때문에 보다 개선된 환자 치료와 outcome을 위해 임상에서 활용 가능. Research: Impact Factor (2010 JCR). The Cochrane Library에서 제공하는 정보의 수준과 적절성에 대해 신뢰할 수 있기 때문에 연구과정에서 활용 가능.  Provide feedback: Cochrane Library에서 제공하는 review와 관련된 정보 혹은 의견을 가지고 계신다면 Cochrane Library상에 Feedback 가능. 8 The Cochrane Library databases
1. The Cochrane Database of Systematic Reviews (Cochrane Reviews) CDSR은 보건 의료  분야의 systematic reviews를 선도하는 리소스로, Cochrane 리뷰 그룹을 통해 준비되어진  모든 리뷰와 프로토콜들이 수록.  각각의 Cochrane 리뷰는 peer-review된 Systematic Review. Cochrane 리뷰그룹 (에디터  팀)에 의해 관리 감독. 월 단위 업데이트. 2. The Database of Abstracts of Reviews of Effects (Other reviews) Cochrane Collaboration에  의해 이행되지 않은 리뷰들의 요약과  수준 평가를  통해 Cochrane의 리뷰를  보완하는 역할.  DARE는 Aabstract와  그 수준에 대한 논평이 담긴 리뷰  요약을 함께 제공하는  데이터베이스.  정책개발 및  의사결정자들을 위한  key resource.  영국 요크대학에  있는 Centre for Reviews and Dissemination (CRD)에 의해 생산.

3. The Cochrane Central Register of Controlled Trials (Clinical trials) MEDLINE이나 EMBASE 등의 서지 데이터베이스 그리고 기타 출판물들로부터 수집된 논문들의 세부내용  제공하는 DB.  3/5정도의 record는  MEDLINE으로부터 수집되어지며, 그 외  EMBASE 등의  데이터베이스와 hand search  결과들이 통합되어 CENTRAL에서 제공.  아티클 제목, 출판정보, 초록까지만 제공.  4. Health Technology Assessment Database (Technology Assessments) 전세계적으로 현재  진행 중이거나 이미 완료된 의료  기술 평가의 세부사항즉 전세계 healthcare intervention의 의학적, 사회적, 윤리적, 경제적  영향에 대한 연구를 제공. HTA 데이터베이스의  목적은 의료 서비스의 품질과 비용  효율성을 개선하는 것으로 INAHTA (International Network of Agencies for Health Technology Assessment) 및 기타 의료 기술 평가 기관에 대한 기관등의 국제적인 네트워크로부터 수집된  정보를 사용해 영국 요크 대학의 CRD에  의해 생산.

5. The Cochrane Methodology Register (Methods Studies) Controlled trials의  운영에 사용된 방법들이 기술된 저널  아티클, 도서, 그리고 컨퍼런스 프로시딩  등 출판물로부터 서지정보를 제공. MEDLINE 그리고 hand search로 수집되며,  systematic review를 준비하는 사람들이 이용할  수 있는 일반적인 방법론적 연구에  대한 정보를 제공.  6. NHS Economic Evaluation Database (Economic Evaluations) NHS EED는 의료중재의  경제적인 평가를 구성하는 구조화된  초록을 제공 의료중재의 quality와 상대적인 장단점을 살펴 전세계 의료 중재의 경제적인 평가를 조직적으로 확인할 수 있도록 하여 의사결정자를 지원.  

11 Number of included protocols and reviews
As of Issue 4, 2011, the CDSR includes 6,602 articles: 4,594 reviews; and 2,008 protocols. Issue 4, 2011 includes  19 new reviews, 37 updated reviews (31 with a new search and 6 with changed conclusions). 31 new protocols and 1 updated protocol. 13 withdrawn reviews and protocols.

13 What is a Systematic Review?
SR 즉 체계적  문헌고찰은  여러 연구결과를  정리 종합하는 과정에서 발생하게 되는 bias를 통제하기  위한 방법. 특정  주제와 연관된 모든 연구들의 체계적인  조합과 비판적인 평가, 그리고 bias를  통제하는 과학적인 방법의 적용이라고  할 수 있음. Narrative  reviews Vs Systematic  reviews: Narrative reviews:  결과를 정리 후 모두 나열. Systematic reviews:  제기된 question에 대한 명확한 서술과  관심의 대상이  되는 하위그룹에  대한 명시, 즉 연관된 연구들  중 적절한 연구결과와 정보를 분석하고  추출해 내는 방법의 차이

14 What is the Cochrane Review?
Cochrane 리뷰는  Cochrane 데이터베이스에 게시되어 있는  health care 및 보건 정책 연구에  대한 SR. Cochrane만의 독특하고 엄격한 절차에  따라 review 되어 지며  아래 세 종류의 SR로 구성 :  1. Health care  및 보건 정책에 사용된 intervention의  benefit과 피해 등을  평가하는 Intervention reviews. 2. 특정질병을 진단 및 발견하는데 있어 진단검사가 얼마나 잘 수행되었는지 평가하는 Diagnostic test의 정확성 reviews. 3. ‘체계적인 문헌고찰과 clinical trial이 제대로 실시되고 보고되었는지‘와  관련된 Methodology reviews.

16 What is a meta-analysis?
일반적으로 비슷한  주제를 연구한다고 해도 각 개별  연구들이 완전히  동일한 조건(독립변수나  트리트먼트의 스케일, 또는 샘플의 수  등)에서  수행될 수 없기  때문에 단순히 각 개별 연구들의  결과를 종합, 비교해서 결론을 내린다면 편의가  발생할 수 있음. 따라서 각 개별  연구들에서 나온 결과를 표준화시킨 후 (이것을 effective size 또는 (eta)라고 부릅니다)  종합 비교할 필요가 있는데 이러한 통계적 분석을 meta analysis라고  함.  * Cochrane의 모든  리뷰가 Meta-analysis를 사용하지는 않음.

현재 Lancet, New England Journal of Medicine, British Medical Journal 등 수많은  저널들은 The Cochrane Library를 SR의 “gold standard” 로 인정 다양한 영역의 의학 저널에 반복적으로 등장 Bias의 개입을 최소화 시키고 신뢰도를 확보하는 엄격한 리뷰 과정으로 기타 다른 리뷰들과 확연히 구분. 최신성 확보와 신뢰도 높은 evidence 제공을 위해 정기적인 업데이트 실시

For  authors  Cochrane 리뷰를  이미 등록한 저자들은 편집 과정  및 데드라인 등에 대한 정보와 지원  내용들을 확인하기 위해 해당 Cochrane  리뷰그룹에 contact. 리뷰 등록에 관심 있는 저자들은 리뷰 등록 과정에 대한 정보를 얻기 위해 역시 해당 관심 분야를 커버하는 Cochrane 리뷰그룹에 contact 해야 함.   미래에 리뷰  등록을 희망하는 저자들은 CDSR상의 해당  주제분야에 본인이  리뷰를 원하는 topic이  이미 프로토콜이나 리뷰자료 형태로  연구 진행 중인지 여부를 먼저 확인해야  함. 리뷰 등록을 위한 더욱 자세한 정보: “How to prepare a Cochrane Review” 를 클릭 (The Cochrane Library 메인 페이지의 Learn 메뉴 아래) 리뷰등록을 위한 소프트웨어, 글로서리, Cochrane 스타일 가이드 등 지원.
