2 견관절의 주된 기능 손과 팔을 신체의 어떤 위치에라도 가져다 놓을 수 있도록 하는 것

3 견갑대(Shoulder girdle)- 5개 관절

4 어깨 관절 운동 관절와 상완관절: 인체에서 움직임이 가장 큰 관절 횡단축 : 굴곡 & 신전운동 시상축 : 내전 & 외전
수직축 : 외회전 & 내회전

5 어깨 관절 운동 _ 내전 대흉근 (Pectoralis major) 광배근 ( Latissimus dorsi)
대원근 (Teres Major) 소원근 (Teres Minor)

11 회전근개 ( Rotator cuff ) 극상근, 극하근, 견갑하근, 소원근 견갑골을 중심으로 상완골을 회전
회전근개 동시 수축 상완 골두를 관절와에 단단히 부착(함요압박) 근육의 균형 (불안정 관절) 관절낭의 안정

12 1. ANTERIOR HUMERUS IND: Trauma, anterior glenoid cavity pain, previous anterior dislocation, restricted Posterior motion, trauma which jams the humerus anterior. PP: Sitting with elbow flexed 135˚ and hand placed on opposite shoulder or behind neck. DP: Standing behind Pt. POC: Both hands are interlocked around elbow of Pt. LOC: Anterior to posterior (toward Dr.)

13 MOD: Dr. interlocks fingers around Pt
MOD: Dr. interlocks fingers around Pt.'s elbow and with his chest he stabilizes the back of the Pt. by getting in close. A: If the humerus is Externally Rotated the choice of position is to place the Pt.'s hand of the opposite shoulder in Internal Rotation B: If the humerus is Internally Rotated the choice of position is to place the Pt.'s hand behind his neck in External Rotation. Traction and thrust toward the Dr. DM: Infraspinatus 2. Teres Minor 3. Rhomboid Major

21 2. SUPERIOR HUMERUS IND: Pain at superior aspect of humerus and/or supraspinatus tendon, bilateral asymmetry of shoulders, restricted inferior humerus motion, trauma which tends to jam the humerus superior. PP: Supine DP: Sitting on side of involvement facing cephalad. CH: Distal humerus. IH: Distal radius and ulna.

22 LOC: Superior to inferior.
MOD: Arch of foot is placed at the proximal humeral head stablizing rib cage and scapula. Correct any internal or external rotation, traction, and thrust. DM: 1. Pectoralis Major (sternal division) 2. Latissimus Dorsi

26 교정법(2) SUPERIOR HUMERUS

27 3. INFERIOR HUMERUS IND: Trauma, pain at inferior glenoid cavity, supraspinatus tendon pain, previous inferior dislocation, restricted superior motion, trauma which jams the humerus inferior. PP: Prone or supine. DP: Side of involvement in a semi-squatting position. CH: Web contact at proximal humerus. IH: Web contanct at distal humerus.

28 LOC: Inferior to superior.
MOD: The pateint is placed prone when the listing includes any external rotation. The patient's involved side is placed in 90˚of elbow flexion and 90˚of shoulder abduction. Traction is made at the proximal humerus in an inferior direction and then thrust. The patient is placed supine when the listing includes any internal rotation. The rest of the procedure is the same as previously described. NOTE: Drawing demonstrates a supine procedure.

POSTERIOR HUMERUS IND: Trauma, pain at posterior aspect of humerus and/or rotator cuff tendons, pain at inferior and/or medial glenohumeral ligaments, previous posterior dislocation, restricted anterior motion, trauma which jams the humerus posterior. PP: Prone with arm at side. DP: Standing on side of involvement. CH: Inferior hand pisiform contact. IH: Superior hand stabilizes contact hand.

37 LOC: Posterior to anterior
MOD: Dr. thrusts with pisiform contact on posterior proximal humerus straight down. DM: 1. Pectoralis Major

40 5. SUPERIOR DISTAL CLAVICLE
IND: Trauma, acromioclavicular separation and/or subluxation, tenderness at involved joint, restricted inferior motion of distal clavicle. PP: Sitting with involved palm of hand on back of neck. DP: Standing behind Pt., Dr. places web of hand over distal clavicle. CH: Web of thumb contacts the distal clavicle IH: Stabilize the elbow joint. LOC: Superior to inferior.

41 MOD: The Dr.'s thrust is made while simultaneously pulling up on the elbow.
DM:1. Pectoralis Major (clavicular division) 2. Anterior Deltoid 3. Subclavius 4. Middle Deltoid 5. Upper Trapezius

O: 후두골의 superior nuchal line의 내측1/3, ligamentum nuchae,C1-C5의 극돌기 I:쇄골의 외측 1/3 N:제11뇌신경과 C3-4의 분지 A:어깨를 올리거나 머리가 같은 방향으로 측굴, 머리와 목 신전

47 6. ANTERIOR PROXIMAL CLAVICLE
IND: Trauma, sternoclavicular joint separation and/or subluxation, tenderness at involved joint, restricted posterior motion of proximal clavicle. PP: Supine. DP: Standing on the opposite side of involvement facing cephalad. CH: Pisiform of superior hand contacts proximal clavicle. LOC: Anterior to posterior. MOD: Dr. pulls Pt.'s arm toward himself while thrusting.
