4 Toll-Like Receptors (1) Cultured nasal epithelial cell 에 10 종류의 TLRs 존 재 (Acta Otolaryngol 2007) CRS 에서 TLR2 가 증가했고, TRL2 와 TLR4 의 발현 이 감소된 경우 수술 후 polyp 재발이 많음 (Am J Rhinol 2006) Polyp-derived fibroblast 에서 TLR2,3,4,5 와 IL-4 가 상보적으로 TARC 를 증가 (ANL 2008)  TARC (thymus and activation-regulated chemokine): 염 증세포에서 주로 분비되며 Th2 cell migration 에 관여

5 Toll-Like Receptors (2) Polyp-derived fibroblast 에서 TLR2,3,4,5 와 IL-4 가 상보적으로 MCP-4 를 증가 (Acta Otolaryngol 2007)  MCP4: chemoattractant for eosinophil Cultured nasal epithelial cell 에 TLR3 의 ligand 인 dsRNA 를 투여하면 RANTES, IL-8, IP-10, GM-CSF 등이 증가

6 Superantigen (1) TCR 와 비특이적 결합을 하여 국소적이지만 광범위 한 T cell 자극 ( 주로 Th2 반응을 유도 ) SAEs 가 검출되는 polyp 의 여러 leukotriene 수치가 검출되지 않는 polyp 보다 증가 (J Am Rhinol 2006) Local Ig production: CRScNP 군의 조직에서 IgA, G, E 및 plasma cell, B cell 이 CSRsNP 및 control 에 비 해 증가해 있었으나 serum 에서는 차이가 없슴 (Clin Exp All 2007)

7 Superantigen (2) Control 과 NP 환자의 middle meatus 에서 S. aureus 의 enterotoxin DNA 는 큰 차이 없슴 (J Am Rhinol 2008) 다양한 Positive vs. Negative Results 다양한 임상 실험 진행 중  3 주간 항생제 치료 후 polyp size 감소  anit-IL-5 후 50% 정도에서 polyp score 호전  anti-IgE 투여 실험 중

8 Fungus CRS 의 원인으로 초기 주목 : fungus 에 대한 allergic 반응으로 이해 Alternaria-specific IgE 와 ECP level 이 polyp 에서 대 조군보다 증가 (Laryngoscope 2007) 정상인에서도 90% 이상 비강에서 fungus 발견됨 Anti-fungal spray, larvage, oral agent 모두 치료 효 과 없는 것으로 결론

9 Allergy polyp 의 발생과 allergy 관계는 아직 불확실 allergy 환자에서 polyp 재발이 흔하다는 보고 많음 eosinophil-dominant with SPT(-) 환자에서 비강스 플레이 후 반응이 noneosinophil-dominant with SPT(+) 환자에서 보다 좋음 (Am J Rhinol 2006) Asthma 를 동반한 polyp 환자에서 ESS 후 anti-IgE (Omalizumab) 을 투여하면 polyp 재발 감소 (Am J Rhinol 2007)

10 Epithelial Ion Channel 호흡기에서의 표면 수분 조절은 주로 Cl - 배출과 Na + 의 흡수를 통해 조절됨 Polyp 의 상당 부분은 수분으로 추정 Cl - secretion channel 의 activity 가 정상에서 보다 polyp 에서 감소함 (Am J Respir Cell Mol Biol 2007) Epithelial Na + channel (ENaC) 으로 인한 Na + 의 흡 수가 polyp 에서 많이 발생 (BBRC 2007)

11 TGF-β Fibroblast 분화 유도 CRScNP 에서 TGF-β 발현이 CRSsNP 보다 3 배 이 상 증가 (Laryngoscope 2005) TGF-β 1 과 TGF-β 2 가 sequential 하게 작용 (Imm Lett 2007) TGF-β 1 이 fibroblast 의 성장 및 collagen, fibronectin 생성 촉진, 하지만 MCP-1, RANTES, eotaxin 등은 발현 감소 (Laryngoscope 2008)  dual effect?

12 MMP (matrix metallo-proteinase) Maintenance of ECM: MMP↔TIMP MMP-2, 9: degradation of collagen IV MMP-9 이 NP 에서 증가 (J Pathol 2001) MMP-2 는 CRScNP 에서, MMP-7 는 CRSsNP 에서, MMP-9 는 양측에서 모두 증가함. TIMP 는 양측 모 두 control 에 비해 감소 (Otolaryngol Head Neck Surg 2008)
