3 Urinary retention(요정체=요폐)
Definition 신장에서 소변이 형성되지만 방광에서 소변을 배출하지 못하는 상태 “inability to empty the bladder when a person voids (micturition) or the accumulation of urine in the bladder because of an inability to urinate.” “a medical emergency that requires prompt recognition and bladder drainage.” Lewis et al.(2014, p. 1092)

Causes 방광출구 폐쇄 (남성 30-50%에서 발생하는 전립선 비대로 요도를 압박, 요도협착, 결석, 종양등으로 인해 발생) 수술로 인한 방광감각 및 방광근육 긴장 감소(척추마취 > 전신마취) 정서적 불안 진통제, 진정제, 항경련제, 베타 교감신경차단제, 향정신약물 치질, 분변매복 등 항문과 직장의 문제 및 회음부 근육 이완방해하는 근경직 부족한 수분섭취 김금순 외(2012) Lewis et al.(2014)

Treatments 내과적 치료: 약물요법- neostigmine(prostigmin) bethanechol(urecholine): 배뇨근 긴장도 올리지만 방광출구, 요도 저항 증가 + phenoxybenzamine(dibenzyline) or prazosin(minipress): 방광출구, 요도 저항감소 (3) Cholinergic medications(콜린성 약물): 방광수축 자극 * 금기: 기계적 폐쇄시 콜린성 약물 투여하면 방광내압 상승으로 소변이 요관으로 역류 혹은 방광 파열이 발생 가능 요도확장술(전신마취 혹은 국소마취 후)- 폐색으로 인한 요정체시 매일 조금씩 더 굵은 도뇨관을 삽입 혹은 요도확장 기구를 이용 도뇨법(단순도뇨, 유치도뇨관 삽입) 김금순 외(2012) Lewis et al.(2014)

Nursing management Avoiding intake of large volumes of fluid over a brief period Drinking small amounts throughout the day Warming up before attempting to urinate Sitting in a tub of warm water or taking a warm shower to help patients urinate Avoiding excessive alcohol intake leading to polyuria and diminished awareness of the need to urinate Drinking a cup of coffee or brewed caffeinated tea to create or maximize urinary urgency Behavior interventions: scheduled toileting every 3-4 hours regardless of the desire to urinate (useful for chronic retention) Perineal care(once or twice a day and when necessary) for patients with urethral catheters Lewis et al.(2014)

15 Urinary reflux(요역류) 신우 용적= 5mL
선천성 기형의 가장 흔한 현상으로 흔히 방광요관 경계에 문제가 생겨 요가 요관을 통해 신우 내로 역류하는 것(Vesicoureteral reflux[방광요관역류]) 신우 용적= 5mL 요도와 방광 연결이 잘못된 경우 요도방광 경계에서도 역류 생길 수 있음 감염(배뇨시 작열감, 발열, 오한, 옆구리 통증)  요관과 요도 부종 및 유착  판막기능 방해  역류 신경근의 기능불량(neuromuscular malfunctions) 방광경의 폐쇄  방광내 압 증가  요관 괄약근 저항력 저하  역류 김금순 외(2012) Lewis et al.(2014)

보존 요법: 아동-사춘기 시기에, 항생제(ampicillin, trimethoprim, sufamethoxazol[Bactrim]), perineal care, 요로 감염 조기 발견하도록 교육 수술요법(방광요관성형술[vesicoureteroplasty], 요관이식술[implantation]): 요 역류로 신장 손상된 성인, 요관과 신우 경계 폐쇄, 치료되지 않는 감염, 성인 이 되어도 요의 없는 경우 요관카테터 삽입: 요관의 지지역활로 치유 촉진, 수술 후 부종으로 인한 폐쇄 방지, 요 배설 김금순 외(2012) Lewis et al.(2014)
