1 Gastric Carcinoid Tumor

2 Case presentation 41/F 평소 건강하던 환자가 개인의원에서 건강검진으로 시행한 내시경검사 및 조직검사에서 이상소견이 발견되어 의뢰됨. 고혈압이나 당뇨 등은 없었고 ,악성질환의 가족력은 없음. 신체검진에서 특이소견이 없음. 41세 여자환자로, 평소 건강하던 환자가 개인의원에서 건강검진으로 시행한 상부위장관 내시경검사상 이상소견이 발견되어 의뢰되었습니다. 고혈압이나 당뇨병등의 가족력은 없었고, 악성질환의 가족력도 없었으며, 신체검진상에서 특이소견 없었읍니다.

3 Endoscopic findings 14335548 서순자
상부위장관 내시경소견입니다. Lower body의 Greater curvature와 anterior wall에 약간 elevated된 fold 의 thickening이 관찰되었습니다.

4 Biopsy findings Synaptophysin 14335548 서순자
Biopsy finding입니다.H&E stain에서 원형 또는 타원형의 핵을 가지는 작고, 균일한 모양의 cell들이 cell nest를 이루고 있는 Carcinoid tumor의 소견입니다.Synpatophysin으로 immunostaining을 시행한 결과 양성반응을 보였습니다.

5 CT findings 복부 CT finding입니다. Stomach에는 별다른 이상소견 보이지 않았으며, liver나 주변 장기, 주변 lymph node에도 metastasis의 소견은 관찰되지 않았습니다.

6 EUS findings (miniprobe, 12 MHz)
서순자 EUS finding입니다. 제 1층에서 기원하는 것으로 보이는 hyperechoic mass의 소견이 관찰되었습니다.

7 Laboratory findings CBC : 5490 – 11.8/37.8 – 251,000 Chemistry
Protein,Total / Albumin : 7.1 g/㎗/ g/㎗ Bilirubin,Total : 0.2 ㎎/㎗ AST / ALT / ALP : 25 U/ℓ / 19U/ℓ / 50U/ℓ Glucose,Fasting : 143 ㎎/㎗ BUN / Creatinine : 11.0 ㎎/㎗ / 0.76 ㎎/㎗ Na / K / Cl : 137 mmol/ℓ / 4.2 mmol/ℓ / 103 mmol/ℓ 24hrs. Urine 5-Hydroxyindoleacetic acid (5-HIAA) : 0.7 mg/day (Ref. range : mg/day) Gastrin (0-90 pg/ml) : 650pg/ml, 1000pg/ml 검사실 소견상, CBC와 blood chemistry에서는 별다른 특이사항 없었으며, 24시간 urine 5-HIAA는 0.7mg으로 정상 이었으며, serum gastrin이 650pg 과 1000pg으로 hypergastrinemia의 소견을 보였습니다.

8 서순자 EMR을 시행하였습니다. Submucosal injection후 submucosal dissection을 시행하였고, 마지막으로 snare로 polypectomy를 시행하였습니다. EMR후 심각한 출혈이나 천공등의 합병증은 없었습니다.

9 Pathology결과 입니다. Tumor 의 size는 0.4x0.2cm이었으며, submucosa까지 extension하였으며, 0.4mm involve하였습니다.

10 서순자 Low power field에서 EGD biopsy와 마찬가지로 작고 둥근 핵을 가진 균일한 세포들이 cell nest를 이루고 있는 carcinoid tumor의 양상을 보여주고 있습니다.

11 서순자 Submucosa부위를 고배율로 관찰한 사진으로, 0.4mm를 involve한 소견을 보여주고 있습니다.

Type I : Carcinoids associated with type A Chronic atrophic gastritis Type II : Carcinoids associated with ZES-MEN-I Type III : Sporadic Carcinoids

13 Treatment of Gastric Carcinoid Tumor
Type I & - II carcinoid tumors Small(<1.0cm), fewer than 2-3 lesions : Endoscopic polypectomy : If recur, antrectomy and local tumor resection Large(>2.0cm) or multiple lesions : Antrectomy and local excision Type III carcinoid tumors Complete or partial gastrecomy (en bloc resection) with local lymph node resection
