It complexes with colipase in the lumen of the small intestine, subsequently binds to triacylglycerol in the presence of bile salts, and catalyses the hydrolytic removal of fatty acid residues to yield free fatty acids and monoacylglycerols. These then become incorporated into bile acid-phospholipid micelles and are absorbed at the brush border of the small intestine, passing into the lymphatic system as chylomicrons. The bile salts are re-circulated, while the fat enters the bloodstream via the thoracic duct and subclavian vein, and is stored in adipose tissue. Xenical irreversibly inhibits gastric, pancreatic and carboxylester lipases, with an IC50 of 120 ng/ml for human duodenal juice and 100 ng/ml for purified porcine pancreatic lipase. The b-lactone of Xenical forms an ester with the side-chain hydroxyl group of a serine in the active site of the lipase, which is stabilised by bile salts. 7 제니칼 - 체내 흡수 0 % 위장관내에만 머물다가 배설 체내 흡수 전혀 안됨 투여량의 97%가 대변으로 배설 .

24 제니칼 Contraindication, Warnings and Precautions
