Initial antibiotic treatment in elderly patients admitted with pneumonia
TREATMENT Most initial therapy for HAP is empirical major factor for antibiotics selection; risk factors for MDR pathogen Risk for MRSA; vancomycin, linezolid Similar to CAP with pseudomonas risk (except macrolide)
Treatment of HCAP (Harrison) Patients without Risk Factors for MDR Pathogens Ceftriaxone (2 g IV q24h) or Moxifloxacin (400 mg IV q24h), ciprofloxacin (400 mg IV q8h), or levofloxacin (750 mg IV q24h) or Ampicillin/sulbactam (3 g IV q6h) or Ertapenem (1 g IV q24h) Patients with Risk Factors for MDR Pathogens 1. Beta-lactam: Ceftazidime (2 g IV q8h) or cefepime (2 g IV q8–12h) or Piperacillin/tazobactam (4.5 g IV q6h), imipenem (500 mg IV q6h or 1 g IV q8h), or meropenem (1 g IV q8h) plus 2. A second agent active against gram-negative bacterial pathogens: Gentamicin or tobramycin (7 mg/kg IV q24h) or amikacin (20 mg/kg IV q24h) or Ciprofloxacin (400 mg IV q8h) or levofloxacin (750 mg IV q24h) plus 3. An agent active against gram-positive bacterial pathogens: Linezolid (600 mg IV q12h) or Vancomycin (15 mg/kg, up to 1 g IV, q12h) Abbreviation: MDR, multidrug-resistant.

Case (60세. M) C/C: 호흡곤란 악화 (D: 1 day ago-) PI: 뇌경색증으로 중환자실 mechanical ventilator 치료 7일째 되는날 갑자기 호흡곤란 악화로, ventilator setting 상 산소요구량 (FiO2)가 증가되었다. Fever와 chills도 있었으며, tracheal secretions이 최근 많아졌다. P/E: Inspiratory crackles on both lower lung field (특히 등 부위에서 저명하게 들림). 양측 폐야의 호흡음 감소는 없음. Chest AP & CT는 다음과 같았다.
