Initial data base Present illness: 환자가 치료를 원하는 문제에 관하여 시간경과에 따라 기술 1) Chronologic disease process. 2) Specific questioning: duration, location, severity, treatment ... 3) Associated symptoms: fever, chill, sweating, headache, weakness ... 4) Known lab. data.

문제목록 (Problem list) 문제목록의 기능 1. 환자의 건강상 문제점을 종합적으로 관리할 수 있는 기준을 제시하는 참고목록( reference lists) 2. 과거 및 현재의 문제를 한꺼번에 파악할 수 있고 문제점 간의 상호관계를 쉽게 파악한다. 3. 진료대상이 되는 ‘문제’ 및 문제의 그 동안의 경과를 쉽게 파악할 수 있게 해주어 의무기록의 index 역할을 한다. 4. 초기자료에서 ‘문제’를 파악하여 최종적으로 정의한 과정을 파악할 수 있게 한다.

문제목록 (Problem list) Stating the problem ‘Problem’은 현재까지 얻을 수 있는 모든 정보에 근거하여 합리적으로 판단할 수 있는 범위 내에서 가장 높은 수준( highest level of refinement)으로 정의되어야. Four levels of recognization 1. Symptom or sign: Epigastric pain 2. Abnormal lab. finding: Microcytic hypochromic anemia 3. Physiologic finding: anemic conjuctiva 4.
