Case : M/46, hematochezia Chief Complaint : Hematochezia Present Illness : 30여년 전 폐결핵으로 완치판정, 1986년 오목가슴으로 수술한 과거력이 있는 자로, 내원 당일 8시간전부터 발생한 복통과 혈변 (3~4차례, 300~400cc)으로 응급실 내원

Past History : DM/HTN/TB/Hepatitis (-/-/+/-) -- old TB, 완치 Social History : Alcohol/smoking (-/-) occupation : FM doctor Past & Current Medical History : OP – pectus excavatum 으로 OP (1986년) BPH (+) - Tamsulosin(Harnalⓡ) recurrent oral ulcer - dexamethasone gargle NSAIDs 간헐적 복용

general weakness (+) headache (-) dizziness (+) Review of Systems general weakness (+) headache (-) dizziness (+) palpitation (-) dyspnea (-) DOE (-) abdominal pain/discomfort (-/-) hematemesis/melena/hematochezia (-/-/+) Physical Examination v/s 100/72 mmHg – 102/bpm – 20/bpm – 36.5℃ m/s alert acutely ill-looking appearance anemic conjunctivae (+) chest : clear breathing sound without rale abdomen : normoactive bowel sound no tenderness/rebound tenderness not palpable mass

Laboratory test CBC 10360 - 10.2 – 30.6% - 268K * Hb change : 10.2 – 8.0 – 5.2 g/dL PT(INR) 1.00 aPTT 33.3 sec ABGA 7.39 - 37- 85 – 22 – 96% Total protein / albumin 7.0/3.9 g/dL AST/ALT 19/16 IU/L BUN/Cr 19.1/0.87 mg/dL

EGD – HD#1 imp) 1. Chronic superficial gastritis,mild,antrum 2. Incomplete study due to food material 3. No evidence of recent bleeding.

Abdomen CT – HD#1 [판독결과] Active bleeding, terminal ileum of right lower abdomen. [결론] Right lower quadrant에 위치한 distal terminal ileum 내의 arterial phase에서 contrast media에 leakage가 관찰되어 active bleeding의 소견임. Wall thickening 이나 mass, pelvic fluid 등의 abnormal finding 없음. Low anterior chest wall에 metalic fixation state 임. Liver, gallbladder, biliary tree, pancreas, both kidney, spleen 정상임. Omentum, mesentery를 포함한 bowel에 other abnormal finding 없으며, pelvic cavity에 이상 소견 없음.

S.M.A Angiography –HD#1 판독결과] Arterial bleeding, branch of ileocolic artery of SMA. [결론] Right femoral artery를 puncture 한 후에 arterial sheath를 insertion 하였음. 그 후 RH catheter를 이용하여서 SMA를 selection 시행한 angiogram상 right lower quadrant에 small contrast extravasation이 의심됨. 그 후 microcatheter를 이용하여서 right colic artery를 selection 해서 시행한 angiogram상 bleeding의 focus는 보이지 않음. Right ileocolic artery를 selection해서 시행한 angigoram상에서 terminal ileum level에 faint한 contrast extravasation이 관찰됨. Microcatheter로 extravasation 되어 있는 artery를 superselection 하였으나 그 artery의 size가 microcatheter보다 더 가늘어서 guide wire는 insertion 되나 catheter는 그 기시부에서 들어가지 않았음.

Colonoscopy –HD#2 Imp) 1. R/O Small bowel bleeding 2. Two ileal erosions [결과] 1. Up to terminal ileum. 2. Two erosions in the terminal ileum. Terminal ileum에 fresh blood가 관찰되나 bleeding focus 없고 erosion에서 출혈 소견 없음. 3. Entire colon에 fresh blood 관찰되나 bleeding focus 관찰되지 않음.

Enteroscopy HD#2 Imp) 1. Ileal Dieulafoy's ulcer bleeding 2. R/O Behcet's disease [결과] Enteroscopic finding 1. IC valve를 지나서 50cm 상방에 exposed vessel 관찰됨. Hemoclip (x6) 시행하여 지혈함. 2. 이 후로 좀 더 진입하여 ileum을 확인하였으나 bleeding focus 관찰되지 않음. 3. Terminal ileum에 2개의 mucosal erosion 관찰됨. 4. Entire colon에 fresh to old blood 관찰되나 bleeding focus로 생각되는 병변은 관찰되지 않음.

Enteroscopy – HD#2

Medical progress HD#1 (Friday) : 1.Hb change : 10.2→8→6.2 g/dL  p-RBC 6u 수혈 후 10.1 g/dL로 상승 2.Procedure emergency duodenoscopy  abdomen CT  angiography 3. ICU admission

Medical progress HD#2 (Saturday) : 1.Hb : 10.1→8.7→6.2 g/dL까지 하강 RBC : p-RBC 2u transfusion, 지혈후 추가 수혈 없이 8.2 g/dL 까지 상승, 유지 2.Procedure colonoscopy (fresh blood in terminal ileum)  small bowel enteroscopy  Hemostasis by clipping

Medical progress 이후 stable vital sign, Hb 보여 ward transfer F/U bleeding : capsule endoscopy  negative 소견 보여 discharge

Final Diagnosis #1. Ileal Dieulafoy’s lesion bleeding #2. r/o Bechet’s disease #3. r/o NSAID induced erosion
